Eligibility for Home Health Care

The eligibility for the services would depend on the qualifications of either from a public health assistance or a private insurance plan. At Bay Nation Home Health, in order to qualify for home health care, the patient must:

• Be under the supervision of a physician
• Need intermittent care
• Require the services of a professional
• Have a reasonable rehabilitation potential
• Be homebound
• Absences are infrequent and of short duration
• Require a taxing effort
